Medication Lockboxes - For CT Residents

Updated Policy Effective April 2024

For Individual Residents of Connecticut:

Medication lockboxes are no longer available for mailing or shipment. All other materials on the Order Form are still available with our standard 4-6-week processing timeline.

For individuals, we are implementing a reservation system for medication lockboxes. Individuals will be limited to a maximum of one lockbox. Lockboxes will be reserved and must be picked up from the Connecticut Clearinghouse at 334 Farmington Avenue, Plainville, CT, 06062.

We appreciate your understanding regarding the change to our shipping policy. Unfortunately, due to the extremely high demand for lockboxes throughout the state and the rising cost of shipping, this option is no longer sustainable. Our program and our funders have decided to prioritize the continued availability of lockboxes and other resources we offer instead of allocating such significant costs to shipping.

Reserving a Medication Lockbox (for individuals only):
  1. Complete the brief reservation form, available below.
  2. Your email address will not be collected. All data collected by the form is completely confidential. Your responses will not be shared publicly; they are only for internal tracking and inventory purposes within our program.
  3. Select your desired container(s) - a limit of 1 of each style of container per resident will be imposed.
  4. Your containers will be reserved for a maximum of EIGHT (8) WEEKS. During this time, the containers can be picked up from the Connecticut Clearinghouse. Pickup is available Monday to Friday, from 9:00am - 4:30pm.
  5. When you come to pick-up your container, please be prepared with your first name, first 3 letters of your last name, and your birthday. This is the first question on the reservation form and is how we will connect you with your reservation. This information will not be used once you have picked up your containers.
  6. At the time of pick-up, you will be asked to a complete a brief, confidential survey. This survey allows us to understand where resources are being distributed and determine what areas of the state may require additional services. The data collected will be used by our program only and will never be publicly available.
  7. If the containers are not picked up within 8-weeks of the reservation, the reservation will be deleted, and you will no longer be guaranteed a container.

Individual CT Resident Lockbox Request Form

Other Ways to Obtain a Safe Storage Container:
  1. If you do not reserve a container, you are able to "walk-in" to the Connecticut Clearinghouse to request a storage container. You can pick-up containers Monday to Friday, from 9:00am - 4:00pm. Reservations will be honored before walk-ins.
  2. The Change the Script Community Resource Van also offers safe storage containers at all of our publicly attended outreach locations. Supplies will be limited in these instances due to the limited storage space available on the van. These supplies will be available on a first-come, first-serve basis at each location.
  3. Regardless of how you request a safe storage container - through the reservation form, walk-in at the CT Clearinghouse, or from the Change the Script Van - you will still be required to complete a brief, confidential survey. This survey allows us to understand where resources are being distributed and determine what areas of the state may require additional services. The data collected will be used by our program only and will never be publicly available.
Additional Options for Lockboxes

The safe storage containers provided by this program are designed to help reduce barriers for CT residents who might not otherwise have access to these resources. IF you have the means to purchase these resources independently, please consider doing so. Your support allows us to continue making these resources available in Connecticut. Please see below for options for purchasing storage containers beyond this program. Please note that any purchases made are at your own discretion.
